Dave Lim
Vestibular Rehabilitation Expert
“BPPV is not the only vestibular disorder. There are many distinct types of dizziness, each requiring a different approach. Dave works closely with other health professionals to reach an accurate diagnosis and deliver the most appropriate, evidence-based solution for each individual condition.”
BPPV is the most common cause of vertigo. It occurs when tiny calcium crystals (otoconia) inside your inner ear become displaced into one of the semicircular canals. When you move your head, these crystals send incorrect signals to your brain about your body's position, triggering brief but intense episodes of spinning and dizziness. It is not dangerous, but it can be very disorienting and significantly affect daily life.
In most cases, BPPV can be resolved in just 1 to 3 sessions. Dave uses the Epley Manoeuvre and other evidence-based repositioning techniques that are highly effective when applied correctly. Most patients notice a significant reduction or complete resolution of symptoms within the first or second visit. Unlike many conditions, BPPV often does not require ongoing treatment — the goal is to resolve it and discharge you as quickly as possible.
Dizziness is a broad term that describes feelings of unsteadiness, lightheadedness, or disorientation. Vertigo is a specific type of dizziness characterised by a false sense of spinning or movement — either you feel like you are moving or the room is moving around you. BPPV-related vertigo is typically triggered by head position changes such as rolling over in bed, looking up, or bending forward. Understanding the distinction is key to accurate diagnosis and effective treatment.
Dave will take a detailed history of your symptoms — when they started, what triggers them, how long each episode lasts, and any associated symptoms like nausea or hearing changes. He will then perform a series of clinical tests, including the Dix-Hallpike test and head impulse test, to identify which canal is affected and confirm the diagnosis. This typically takes 30 to 45 minutes. You will leave with a clear diagnosis and a treatment plan.
Vestibular rehabilitation is very safe when performed by a trained physiotherapist. The repositioning manoeuvres used for BPPV may briefly intensify your dizziness during the procedure, but this is a normal and expected part of the treatment — it means the crystals are being moved into the correct position. Dave will explain every step beforehand and ensure you are comfortable throughout. There are no medications or invasive procedures involved.
Yes, BPPV can recur. Studies suggest a recurrence rate of around 15 to 50% within 1 to 5 years, particularly in individuals with a history of head trauma, osteoporosis, or inner ear conditions. However, recurrence is usually straightforward to treat again. Dave will also teach you a home repositioning exercise (Brandt-Daroff exercises) so that if mild symptoms return, you have the tools to manage them independently.
No referral is required. You can book directly with Core A Physio for a vestibular assessment. However, if your symptoms include significant hearing loss, sudden severe headache, double vision, difficulty swallowing, or weakness in your limbs, please seek emergency medical attention first, as these may indicate a neurological cause that requires immediate assessment by a doctor.
Core A Physio is built on the belief that exceptional physiotherapy requires more than clinical skill — it demands genuine care, clear communication, and a commitment to lasting results.
